Adds `messages.responseUsage` (precedence session -> channel -> config default
-> off) so the persistent /usage footer can default-on, with three distinct
states: explicit on (tokens/full), explicit off (persisted), and unset (inherit
the configured default).
Unifies effective-value resolution behind a single channel-aware resolver
`resolveEffectiveResponseUsage` used by reply rendering, the no-arg /usage
toggle, the ACP control, and the gateway session-row builder; the row builder's
`effectiveResponseUsage` is carried through sessions.changed events, chat
snapshots, and the UI row so live consumers never go stale. `/usage reset`
(aliases inherit/clear/default) clears the override to inherit; only explicit
off persists; a full session reset preserves the preference. ACP "Usage detail"
gains an "inherit" option for unset sessions. Docs/help/completions updated; "on"
documented as a legacy alias; config-doc baseline regenerated.
Persistent ACP threads died on the second turn for Kiro: when the backend
can no longer resume a stale session, acpx raises a SessionResumeRequiredError
whose reason text varies by backend ("Resource not found" for Claude,
"Internal error" / RequestError -32603 for Kiro). The recovery gate matched
the human reason text and required "resource not found", so Kiro's "Internal
error" never triggered the fresh-session retry and the thread produced no
reply (ACP_TURN_FAILED).
Recover by acpx's structured detail code instead of the reason text: acpx
tags every such failure with detailCode "SESSION_RESUME_REQUIRED"
(retryable), independent of wording. The two AcpRuntimeError construction
seams were discarding detailCode, so preserve it on AcpRuntimeError and match
it across the error and its cause chain. This fixes every backend's
resume-required failure and is more precise than the reason regex — a generic
"Internal error" without the code is still surfaced rather than silently
retried.

Preserve task and TaskFlow durability by persisting before in-memory registry mutation and surfacing explicit persistence failures instead of reporting fake success.
Adds non-throwing try-create runtime helpers while keeping existing throwing public create APIs compatible. Maintainer follow-up keeps task/TaskFlow sync repair bounded, prevents split task/delivery-state writes, and keeps CI green on the current base.
