A device retrying with a broken token minted a new pending requestId (and
approval alert broadcast) every 5-minute TTL window because refresh preserved
ts and expiry keyed on ts. Pending requests now stay alive via an internal
refreshedAtMs keepalive (ts still owns ordering/--latest), superseded device
requests broadcast device.pair.resolved like node pairing already did, and the
Mac app keeps one alert per device, closes superseded visible alerts, and
resyncs its queue when approving a stale request.
Closes #100974

before wrapping with wrapExternalContent. The agent media extractor scans every
browser tool-result text block via splitMediaFromOutput which treats line-start
MEDIA: as a trusted local-media delivery directive, and browser is on the
trusted-media allowlist. Without neutralization, page or vision-provider output
containing 'MEDIA:/tmp/secret.png' could synthesize a channel-deliverable media
artifact from untrusted content. wrapExternalContent itself does not strip
line-start directives. Introduce neutralizeMediaDirectives in vision.ts that
prepends '[neutralized] ' to any line whose trimStart() begins with MEDIA:
(case-insensitive), defanging the parser anchor while keeping the original
text human-readable.
P2 (compatibility): pass resolveRuntimeImageSanitization() to imageResultFromFile
in the vision-failure catch fallback. The non-vision screenshot path already

Per docs/CLAUDE.md, Mintlify anchor generation is brittle for headings
that contain em dashes, apostrophes, and ampersands. Normalize 9 H2
headings across docs/date-time.md, docs/pi.md, docs/platforms/{index,
macos, mac/webchat, mac/peekaboo}.md, docs/nodes/{images,audio}.md, and
docs/reference/AGENTS.default.md from `X & Y` to `X and Y` so anchors
do not collapse on entity decoding. Verified no inbound anchor references
to the renamed sections in the docs tree.

- This PR adds WebChat server-side dictation through a new authenticated `chat.transcribeAudio` Gateway RPC, MediaRecorder composer controls, docs/changelog updates, and focused gateway/UI tests.
- Reproducibility: yes. Current main reproduces the missing feature by inspection: the Gateway method list, write scopes, docs, and WebChat voice-control test have no `chat.transcribeAudio` server-dictation path.
